Job Summary
Implement residents' personal care plans while focusing on their strengths, preferences, and routines to provide comfort, companionship, and belonging. Deliver therapeutic interventions supporting physical, spiritual, emotional, and clinical needs across multiple diagnostic groups, including urinary incontinence, vestibular rehab, and home safety assessments. Complete documentation to meet reimbursement and regulatory requirements within a skilled nursing setting. Work part-time on a PRN basis, ideally 1 to 2 weekdays per week, with onsite physicians and medical centers available for wellness and sick care.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
