Job Summary
Implement residents' personal care plans while focusing on their strengths, preferences, and routines. Provide a holistic approach supporting physical, spiritual, emotional, and clinical needs across multiple diagnostic groups including urinary incontinence, vestibular rehab, home safety assessment, and movement disorders. Complete documentation to meet reimbursement and regulatory requirements. Work within an interdisciplinary clinical team to ensure resident needs are addressed throughout their continuum of care at Ann's Choice by Erickson Senior Living.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
