Job Summary
Implement residents' personal care plans while focusing on their strengths, preferences, and routines to provide comfort and companionship. Complete documentation to meet reimbursement and regulatory requirements. Provide therapeutic support for older adults across multiple diagnostic groups within a skilled nursing setting. Work onsite with an interdisciplinary clinical team to address resident needs throughout their continuum of care. No additional travel required as all clients live within the community.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
