Job Summary
Implement residents' personal care plans while focusing on their strengths, preferences, and routines to support physical, spiritual, emotional, and clinical needs. Complete documentation to meet reimbursement and regulatory requirements. Provide therapeutic services to older adults across multiple diagnostic groups within a skilled nursing setting. Work onsite with the interdisciplinary clinical team to address resident care needs throughout their continuum.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
