Job Summary
Implement residents' personal care plans while focusing on their strengths, preferences, and routines to provide comfort and companionship. Deliver holistic support addressing physical, spiritual, emotional, and clinical needs across multiple diagnostic groups, including urinary incontinence and vestibular rehab. Complete documentation to meet reimbursement and regulatory requirements within a skilled nursing setting. Work 1 to 2 weekdays per week with onsite physicians and an interdisciplinary clinical team. No additional travel required as all clients reside on the campus.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
