Job Summary
Per Diem Physical Therapist needed to join in-house rehab team serving residents with person-centered care. Responsibilities include consulting with patients, assessing evaluations and test results, developing and administering personalized rehabilitation programs and treatment plans, ensuring therapy aligns with the plan of care, monitoring patient responses and adjusting approach with IDT collaboration, providing clinical support and education to patients and families, collaborating with interdisciplinary team to ensure quality care, thorough and timely documentation meeting regulatory requirements, and attending mandatory meetings. New grads welcome; flexible schedules and in-house rehab offered.
Required Qualifications
- Active/Valid Texas license in Physical Therapy
- In good standing with all regulatory agencies and licensing boards
- SNF/Long term care experience preferred
- Current knowledge of treatment practices
- Knowledge of Net Health Electronic Documentation System preferred
