Job Summary
Per Diem (PRN) Physical Therapist needed to join in-house rehab team providing person-centered care to residents. Responsible for consulting with patients, assessing evaluations, developing and implementing personalized rehabilitation plans, administering PT treatments, monitoring responses, collaborating with the interdisciplinary team, ensuring documentation compliance, attending Director of Rehab meetings, and providing education to patients and families. Requires Texas PT license; prior SNF/long-term care experience preferred; in-house rehab environment; flexible schedules; competitive compensation.
Required Qualifications
- Active/Valid Texas license in Physical Therapy
- In good standing with all regulatory agencies and licensing boards
