Job Summary
Provide per diem Physical Therapist services in a SNF/long-term care setting. Responsibilities include consulting with patients to assess physical condition, developing and implementing personalized rehabilitation plans, administering prescribed therapies to relieve pain and improve function, monitoring responses and modifying treatment with IDT collaboration, and ensuring thorough documentation in accordance with the plan of care and regulatory requirements. Collaborate with the interdisciplinary team to ensure quality patient care, participate in required meetings, and support patients, family members, and caregivers through education. Flexible schedules with in-house rehab opportunities and an emphasis on delivering person-centered care are highlighted, with New Grads welcome to apply.
Required Qualifications
- Active/Valid Texas license in Physical Therapy
- In good standing with all regulatory agencies and licensing boards

- SNF/Long term care experience preferred
- Knowledge of Net Health Electronic Documentation System preferred
