Job Summary

Evaluate patients, establish treatment plans with functional goals, and progress therapy to restore function or prevent disability. Provide periodic written communication to physicians, manage discharge, and follow up with patients. Supervise rehabilitation aides and physical therapist assistants as needed per state practice acts. Document all treatments, progress notes, and insurance forms in a timely manner. Build professional relationships with referral sources including physicians, PAs, and case managers. Accompany patients to physician visits and host community events such as health fairs or in-services. Maintain state licensure and complete all compliance standards.

Required Qualifications

  • Degree from an accredited Physical Therapy Program
  • Current Professional licensure as a Physical Therapist
  • Current CPR Certification
  • Ability to read, write/type and speak English proficiently
  • Ability to see at close range, distance vision, peripheral vision, depth perception, and the ability to adjust focus
  • Ability to speak and understand
  • Ability to fulfill any office activities normally expected in an office setting, to include, but not limited to: remaining seated for periods of time to perform computer based work, participating in filing activity, lifting and carrying office supplies (paper reams, mail, etc.)
  • Ability to stand, walk, sit, use hands to feel, and reach with hand and arms
  • Occasionally lift and/or move up to 20‐25 pounds
  • Fine hand manipulation (keyboarding)
  • Maintain state license
  • Complete all compliance standards as requested
