Job Summary
As a Physical Therapist (PT), you will provide home-based therapy care for patients directed by attending physicians while part of an interdisciplinary team focused on compassionate quality care. You will educate patients and their families, document treatment and progress, and develop individualized care plans, ensuring compliance with all legal guidelines. The role requires a current PT license, a minimum of one year of acute care experience, and strong communication skills. Responsibilities include performing assessments, treating patients using various techniques, and promoting continuity of care.
Required Qualifications
- Physical Therapist with a current license in the state of employment.
- Minimum of one year of experience as a PT in an acute setting is required.
- Valid driver’s license and auto insurance with your name as a listed driver.
Desired Qualifications
- Home care experience is desired.
- Knowledgeable on therapy best practices.
- Ability to make appropriate physical therapy judgments.
- Ability to identify a situation and handle it with the best possible solution.
Additional Requirements
