Job Summary
Physical Therapist (PT) PRN role in Home Health serving Shelby County, TN and surrounding areas. Provide PT services according to a physician’s plan of care and state practice guidelines; evaluate patient function and help develop individualized care plans; educate patients, families, and caregivers on treatment plans and activities; translate prescribed exercises into functional activities and activities of daily living; recommend equipment and home-environment modifications to support safety and independence; collaborate with physicians and interdisciplinary team members; document clinical notes, progress reports, and summaries; supervise Physical Therapist Assistants per policy and state regulations; maintain accurate records and track patient goals and outcomes. Flexible PRN schedule with Monday–Friday as needed, along with mileage reimbursement and comprehensive benefits.”} } } }{
Required Qualifications
- Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
- Current Physical Therapist (PT) license in the state of practice
- Current CPR and First Aid certification
- Reliable transportation, valid driver’s license, and current automobile insurance
