Job Summary
Physical Therapist to evaluate patients and deliver a broad range of therapeutic interventions focused on rehabilitating physical and developmental disorders. Role is hands-on, collaborative, and impactful—involving clinical problem-solving, data interpretation, and documentation in an EPIC system. Responsibilities include evaluating and treating patients across inpatient, outpatient, long-term care, community-based care, and home health settings; coordinating a comprehensive rehabilitation program to meet physical, psychosocial, and environmental needs; promoting patient/caregiver self-determination; facilitating discharge planning; and advocating for patients. Requires knowledge of physical therapy principles, basic computer skills, and understanding of applicable regulations. Qualifications include a Bachelor’s degree in Physical Therapy; Doctorate of Physical Therapy preferred.
