Job Summary
Physical Therapist needed for in-home rehabilitation in Rochester, NY. Responsibilities include initial and ongoing assessments, planning and delivering therapy across strength, balance, gait, and mobility training, educating patients and caregivers on exercises, safety, and equipment use, monitoring progress, adjusting care plans, and collaborating with nurses, OTs, aides, and social workers. Requires current PT license, DPT or equivalent, and at least 1 year of relevant experience; valid driver's license and CPR. Flexible schedule with a patient-centered, high-impact home healthcare focus.
Required Qualifications
- Current Physical Therapist license
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
- Minimum of 1 year experience in home health, rehab, or hospital setting preferred
- CPR certification and compliance with health requirements
- Valid driver’s license and reliable transportation
