Job Summary
Evaluate and treat older adults in their private residences or independent living facilities by developing and delivering individualized physical therapy programs. Screen, examine, and assess patients to establish diagnoses, identify impairments, and design treatment plans that restore function and promote independence. Instruct patients and caregivers in essential skills, including therapeutic exercise, manual therapy, and adaptive device training, while performing routine reexaminations to modify care. Supervise Physical Therapy Assistants and students, consult with the rehabilitation team to address comprehensive needs, and ensure adherence to clinical documentation and safety standards. Maintain equipment, order supplies, and conduct home mobility assessments to recommend safe community navigation.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
