Job Summary
Evaluate and treat older adults in home and independent living facility settings by screening, examining, and developing individualized physical therapy programs. Implement therapeutic techniques including aerobic conditioning, gait training, manual therapy, and adaptive device application while instructing patients and caregivers to promote independence. Supervise therapy assistants and support staff, consult with the rehabilitation team to coordinate comprehensive care, and perform home mobility assessments to ensure safe community reintegration. Adhere to clinical documentation, billing requirements, and safety protocols while adapting treatment plans based on patient progress.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- Valid driver's license or reliable, independent transportation to and from the patients' residence
- Must be able to lift 50 lbs
