Job Summary
Evaluate and treat older adults in home and independent living facility settings by developing, implementing, and modifying individualized physical therapy programs. Screen, examine, and assess patients to establish diagnoses, identify impairments, and design treatment plans covering therapeutic exercise, manual therapy, and functional training. Instruct patients and caregivers in skills essential for independence while managing referrals, safety hazards, and adaptive device applications. Supervise therapy assistants and support personnel, ensure accurate clinical documentation and billing, and contribute to staffing conferences. Maintain equipment, order supplies, and perform routine reexaminations to progress care plans.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
