Job Summary
Evaluate and treat older adults in their private residences or independent living facilities through outpatient rehabilitation services. Screen, examine, and diagnose impairments to establish treatment goals, then implement individualized programs including therapeutic exercise, manual therapy, and functional training. Instruct patients and caregivers in skills to promote independence while managing referrals, safety hazards, and adaptive device applications. Maintain clinical documentation, attend staffing conferences, and supervise therapy assistants or students as required.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
