Evaluate and treat older adults in private residences and independent living facilities by developing, delivering, and modifying individualized physical therapy programs. Screen, examine, and assess patients to establish diagnoses, determine impairment levels, and design treatment goals while instructing patients and caregivers in skills to promote independence. Implement therapeutic techniques including exercise, manual therapy, and mobility training; order supplies; and adapt plans based on patient progress. Supervise support personnel as needed and collaborate with other disciplines to address comprehensive patient needs. Ensure adherence to documentation requirements, safety standards, and department schedules.
