Job Summary
Evaluate, diagnose, and treat older adults in their private residences or independent living facilities by developing and implementing individualized physical therapy programs. Conduct comprehensive screenings, including history reviews and functional assessments, while instructing patients and caregivers in mobility skills, adaptive device use, and chronic disease management. Administer therapeutic exercises, manual therapy, and modalities to restore independence, and collaborate with the rehabilitation team to make referrals and adapt care plans. Supervise therapy assistants and support personnel as needed, while ensuring timely documentation, safety compliance, and equipment maintenance. Maintain a flexible schedule with a 35-hour week, utilizing a driver's license for travel to patient homes.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- Valid driver's license or reliable, independent transportation to and from the patients' residence
- Must be able to lift 50 lbs
