Job Summary
Provide outpatient rehabilitation services in the home for older adults, evaluating patients, developing individualized therapy programs, and instructing patients and caregivers in skills to promote independence. Screen, examine, and treat impairments using therapeutic exercise, manual therapy, and modalities while adapting plans based on patient progress. Supervise assistants and students, consult with the rehabilitation team, and manage referrals to address comprehensive patient needs. Ensure adherence to clinical documentation, safety protocols, and equipment maintenance standards. Maintain a flexible schedule and hold a valid state license with a Master's degree or equivalent experience.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- Valid driver's license or reliable, independent transportation to and from the patients' residence
