Job Summary
Evaluate and treat older adults in home-based settings, including private residences and independent living facilities. Assess patient history, conduct examinations, and design individualized therapy programs to restore function and promote independence. Implement therapeutic techniques such as gait training, manual therapy, and electrotherapeutic modalities while instructing patients and caregivers on mobility skills and chronic disease management. Supervise Physical Therapy Assistants and students, coordinate referrals to other disciplines, and maintain comprehensive clinical and billing documentation. Ensure patient safety by identifying hazards and enforcing safety protocols, while adapting treatment plans based on ongoing reexaminations and patient progress.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
