Job Summary
Evaluate and treat older adults in home and independent living settings, developing and delivering personalized physical therapy programs to restore function and promote independence. Conduct comprehensive screenings, including history, systems review, and functional assessments, to establish diagnoses, identify impairments, and design tailored treatment plans involving therapeutic exercise, manual therapy, and adaptive device training. Instruct patients and caregivers in mobility skills and safety interventions, while managing referrals to other disciplines and overseeing Physical Therapy Assistants and students. Maintain accurate clinical and billing documentation, adhere to department schedules, and ensure a safe environment by monitoring equipment and addressing fire hazards.
Required Qualifications
- valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- valid driver's license or reliable, independent transportation to and from the patients' residence
