Job Summary
Provide outpatient physical therapy services in home and independent living facility settings for the older adult population. Evaluate patients, develop individualized treatment programs, and implement therapeutic techniques including exercise, manual therapy, and mobility training. Instruct patients and caregivers on skills to promote independence while managing chronic disease and fall risks. Supervise therapy assistants, students, and support personnel as needed. Maintain accurate clinical and billing documentation, attend care conferences, and consult with the rehabilitation team to coordinate referrals. Order and maintain equipment while ensuring a safe environment.
Required Qualifications
- valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- valid driver's license or reliable, independent transportation to and from the patients' residence
- Must be able to lift 50 lbs
