Job Summary
Evaluate and treat older adults in their homes or independent living facilities through outpatient rehabilitation services. Assess patient needs, develop individualized therapy programs, and implement treatment plans covering therapeutic exercise, manual therapy, airway clearance, and functional training. Instruct patients and caregivers on skills to promote independence, while making referrals to other disciplines for comprehensive care. Supervise Physical Therapy Assistants, students, and support personnel as applicable. Maintain accurate clinical and billing documentation, attend staffing conferences, and ensure a safe environment by managing equipment and identifying hazards.
Required Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Must have a Master's degree in Physical Therapy
- Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy
- Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy
- Must have a valid driver's license or reliable, independent transportation to and from the patients' residence
