Job Summary

The Physical Therapist will provide outpatient rehabilitation in-home for older adults, evaluating and treating patients in their homes or in independent living facilities. Responsibilities include developing and delivering individualized therapy programs, educating patients and caregivers, coordinating referrals to other disciplines, supervising PT assistants and students as needed, maintaining timely clinical and billing documentation, and collaborating with the rehabilitation team to optimize patient outcomes. Treatment modalities encompass therapeutic exercise, gait and balance training, manual therapy, airway clearance techniques, neuromuscular re-education, functional training, and home/workplace safety planning. The role supports a flexible schedule and involves travel to patients’ residences as part of Powerback Rehabilitation To You services. Qualifications include a valid PT license, a master’s degree in PT (or equivalent), and the ability to work within Home/Community settings; driving access is required. Required Qualifications

  • Valid Physical Therapy license in state(s) of practice
  • Master's degree in Physical Therapy; or Master's in a Physical Therapy related field plus Bachelor's in Physical Therapy

  • Foreign-educated PT degree evaluated for US equivalence
  • Valid driver’s license or reliable transportation to patients’ residences
