Job Summary
Physical Therapist responsible for in-home outpatient rehabilitation services for older adults, delivering patient-centered care in private residences or independent living facility settings. Responsibilities include evaluating and treating patients, developing and adjusting individualized therapy programs, coordinating with the rehabilitation team, educating patients and caregivers, supervising PT Assistants and students as needed, and ensuring timely clinical and administrative documentation. Services cover therapeutic exercise, gait and locomotion training, manual therapy, functional training, airway clearance techniques, orthotics/prosthetics fitting and training, wellness education, falls risk assessment, and case management in home and community settings. Requirements include a valid PT license, a Master's degree in Physical Therapy (or equivalent), and eligibility for licensure or credential evaluation if earned outside the US; a valid driver’s license or reliable transportation is also required. The role offers a posted hourly salary range with a bonus and supports H-1B visa sponsorship and relocation assistance.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy; or Master's in PT-related field in addition to a Bachelor's degree in Physical Therapy; or Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- Foreign PT degree must be evaluated to determine US equivalency to a PT degree
- Valid driver's license or reliable, independent transportation to and from patients' residences
