Job Summary
Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, memory care) with evaluations, individualized plans of care, and evidence-based treatment. Deliver therapy sessions that support mobility, independence, safety, and aging-in-place goals. Document within the EMR per regulatory standards; collaborate with an interdisciplinary team (PT/OT/SLP/Fitness Specialists); educate patients, caregivers, and staff; and offer telehealth/virtual care when appropriate to extend access and flexibility. Role involves in-home and senior living settings, opportunities to tailor your schedule, and autonomy in clinical decision-making.
Required Qualifications
- Licensed Physical Therapist in NY state (or eligibility to obtain)
- Degree from an accredited Physical Therapy program
- Current CPR certification
- Proficiency with EMR systems
