Job Summary
Assess patient needs, develop and deliver physical therapy programs, and implement individualized treatment plans to restore and enhance performance in outpatient or in-home settings. Screen, examine, and evaluate patients by reviewing history, applying tests, and establishing diagnoses while documenting all clinical and billing information. Supervise Physical Therapy Assistants, students, and support personnel according to state licensure requirements. Educate patients and caregivers on independence, safety, and adaptive device use, while making referrals to other disciplines for comprehensive care. Maintain adherence to department schedules, attend care and staffing meetings, and ensure a safe environment free of hazards. Perform home and community mobility assessments, manage fall risks, and incorporate health and wellness programming into treatment plans.
Required Qualifications
- Valid Physical Therapy license in the state of practice (or proof of license eligibility)
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy-related field plus a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy with five years of progressively responsible PT experience
- Foreign PT degrees must be evaluated as equivalent to a U.S. Physical Therapy degree
- Must possess a valid driver's license or reliable transportation to travel to patient residences
- Strong organizational skills
- Effective time management
- Strong written and verbal communication skills
