Job Summary
Assess patient needs and deliver individualized physical therapy programs to restore, reinforce, and enhance performance in home or independent living facility settings. Screen, examine, and evaluate patients by reviewing history, applying tests, and interpreting findings to establish diagnoses and treatment goals. Implement therapeutic exercises, manual therapy, and adaptive device training while educating patients and caregivers to promote independence and safety. Supervise therapy assistants and students, maintain clinical and billing documentation, and consult with the rehabilitation team to adapt plans as necessary. Ensure compliance with state licensure, HIPAA regulations, and safety standards while performing mobility assessments and fall risk prevention.
Required Qualifications
- Valid Physical Therapy license in the state of practice (or proof of license eligibility)
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy-related field plus a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy with five years of progressively responsible PT experience
- Foreign PT degrees must be evaluated as equivalent to a U.S. Physical Therapy degree
- Must possess a valid driver's license or reliable transportation to travel to patient residences
- Strong organizational skills
- Effective time management
- Strong written and verbal communication skills
