Job Summary
Conduct outpatient physical therapy services in patients' private residences or independent living facilities, evaluating needs, developing individualized programs, and restoring functional performance. Supervise Physical Therapy Assistants, students, and support personnel while adhering to state licensure and clinical documentation standards. Implement therapeutic exercises, manual therapy, and adaptive device training to promote independence, safety, and community mobility. Consult with the rehabilitation team to make referrals and modify treatment plans based on patient progress. Maintain a safe environment by driving to patient locations and reporting equipment issues.
Required Qualifications
- Valid Physical Therapy license in the state of practice (or proof of license eligibility)
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy-related field plus a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy with five years of progressively responsible PT experience
- Foreign PT degrees must be evaluated as equivalent to a U.S. Physical Therapy degree
- Must possess a valid driver's license or reliable transportation to travel to patient residences
- Strong organizational skills
- Effective time management
- Strong written and verbal communication skills
