Job Summary
Conduct outpatient rehabilitation services in patients' private residences or independent living facilities, delivering therapy in functional environments. Perform comprehensive evaluations, establish diagnoses, and implement individualized physical therapy programs including therapeutic exercise, manual therapy, and mobility training. Supervise Physical Therapy Assistants and students while coordinating with interdisciplinary teams and managing patient care conferences. Maintain documentation standards, billing timelines, and equipment inventory. Reassess patients regularly to modify treatment plans and educate patients and caregivers on chronic disease management and home safety. Requires a valid Physical Therapy license, Master's degree or equivalent experience, and ability to lift up to 50 pounds. Offers a flexible schedule with varied work settings.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice or proof of eligibility
- Master's degree in Physical Therapy
- Master's degree in a Physical Therapy–related field plus a Bachelor's degree in Physical Therapy
- Bachelor's degree in Physical Therapy with five (5) years of progressively responsible experience
- International degrees must be evaluated for U.S. equivalency
- Valid driver's license and reliable transportation to patient residences
- Strong organizational and time management abilities
- Effective verbal and written communication skills
- Ability to lift, push, pull, and carry up to 50 pounds
- Regular bending, kneeling, stooping, climbing stairs, balancing, and working on uneven surfaces
- Exposure to noise, moving machinery, environmental hazards, and automotive equipment
- Occasional use of protective equipment (gloves, gowns, masks)
