Physical Therapist - Per Visit

Develop a patient-centered plan of care with input from patients, addressing their safety, educational, and developmental needs. Interpret clinical data to set and revise goals while explaining treatment techniques and outcomes to patients and families. Recommend therapeutic procedures and modalities, address home exercise programs, and maintain an adequate caseload adhering to productivity standards. Comply with documentation standards and communicate regularly with supervisors, team members, and physicians. Attend continuing education seminars and present educational programs to staff. Administer physical therapy services under medical prescription in accordance with Hospital regulations and the State of Connecticut Practice Act.
