Physical Therapist - Per Visit

Develop a plan of care for each patient incorporating their safety, educational, and developmental needs. Interpret and assess patient data through goal setting and revision while explaining treatment plans to patients and families. Recommend therapeutic procedures and modalities, address home exercise programs, and maintain an adequate caseload adhering to productivity standards. Comply with professional documentation standards and communicate regularly with supervisors, team members, and referring physicians. Attend continuing education seminars and present educational programs to staff. Administer physical therapy services under medical prescription in accordance with Hospital regulations, State of Connecticut Practice Act, and quality standards.
