Job Summary
Develop a patient-centered plan of care incorporating safety, educational, and developmental needs while interpreting assessment data to set and revise goals. Administer therapeutic procedures and modalities, address home exercise programs, and explain treatment outcomes to patients and families. Comply with documentation standards, maintain an adequate caseload meeting productivity targets, and communicate regularly with supervisors, team members, and referring physicians. Attend required continuing education seminars and present educational programs to staff. Work per visit in Middletown, CT, with days, 1st shift availability.
Required Qualifications
- Graduate of an accredited School of Physical Therapy
- Licensed or eligible for licensure to practice in the State of Connecticut
Desired Qualifications
- Experience with electronic medical charting
- 3-5 years clinical experience
- Flexibility with scheduling
