Physical Therapist - Per Visit

Develop a personalized plan of care for each patient, incorporating safety, educational, and developmental needs while interpreting assessment data to set and revise goals. Administer therapeutic procedures and modalities, address home exercise programs, and maintain an adequate caseload adhering to departmental productivity standards. Provide patient and family education on treatment techniques and outcomes, ensuring compliance with professional documentation standards. Communicate regularly with the Rehabilitation Supervisor, team members, and referring physicians to coordinate patient care. Maintain licensure in Connecticut and attend required continuing education seminars.
