Physical Therapist - Per Visit

Job Summary
Administer Physical Therapy services under medical prescription in accordance with Hospital regulation, State of Connecticut Practice Act, and quality standards, including clinical oversight of PTAs. Develop individualized plans of care with patient input, interpret assessment data to set and revise goals, and recommend therapeutic procedures and modalities. Address home exercise programs, maintain caseloads adhering to productivity standards, and comply with professional documentation. Communicate regularly with supervisors, team members, and referring physicians. Attend continuing education seminars and present educational programs to staff. Work per visit in Middletown, CT, with days 1st shift availability.
Required Qualifications
- Graduate of an accredited School of Physical Therapy
- Licensed or eligible for licensure to practice in the State of Connecticut
Desired Qualifications
- Experience with electronic medical charting
- 3-5 years clinical experience
- Flexibility with scheduling
