Physical Therapist - Per Visit

Develop a plan of care for each patient incorporating their safety, educational, and developmental needs. Interpret patient data through goal setting and revision while explaining treatment techniques and outcomes to patients or families. Recommend therapeutic procedures, address home exercise programs, and maintain an adequate caseload adhering to productivity standards. Comply with documentation standards and communicate regularly with the Rehabilitation Supervisor, team members, and referring physicians. Administer Physical Therapy services under medical prescription in accordance with Hospital regulations and the State of Connecticut Practice Act. Attend continuing education seminars and present educational programs to Rehab staff or medical professionals.
