Physical Therapist - Per Visit

Develop a plan of care for each patient incorporating their safety, educational, and developmental needs. Interpret assessment data to set and revise goals, explaining treatment techniques and outcomes to patients and families. Recommend therapeutic procedures and modalities while addressing home exercise programs. Comply with documentation standards and maintain an adequate caseload adhering to productivity metrics. Communicate regularly with supervisors, team members, and referring physicians. Attend continuing education seminars and present educational programs to staff. Administer physical therapy services under medical prescription in accordance with Hospital regulations, the State of Connecticut Practice Act, and quality standards.
