Physical Therapist - Per Visit

Develop a patient-specific plan of care incorporating safety, educational, and developmental needs while interpreting assessment data to set and revise goals. Administer therapeutic procedures and modalities, address home exercise programs, and communicate regularly with supervisors, team members, and referring physicians. Comply with professional documentation standards and maintain an adequate caseload adhering to departmental productivity metrics. Complete required continuing education seminars and present educational programs to Rehab staff. Requires a Connecticut-licensed Physical Therapist with 3-5 years of clinical experience and flexibility with scheduling.
