Job Summary
Develop a patient-centered plan of care incorporating safety, educational, and developmental needs while administering physical therapy services under medical prescription. Interpret assessment data to set and revise goals, explain treatment techniques to patients and families, and address home exercise programs. Maintain an adequate caseload adhering to productivity standards, comply with documentation requirements, and communicate regularly with supervisors, team members, and referring physicians. Present educational programs to Rehab staff and attend continuing education seminars. Work per visit shifts in Middletown, CT, with flexibility for scheduling.
Required Qualifications
- Graduate of an accredited School of Physical Therapy
- Licensed or eligible for licensure to practice in the State of Connecticut
Desired Qualifications
- Experience with electronic medical charting
- 3-5 years clinical experience
- Flexibility with scheduling
