Physical Therapist - Per Visit

Develop a plan of care for each patient incorporating safety, educational, and developmental needs while interpreting assessment data to set and revise goals. Administer therapeutic procedures and modalities under medical prescription, addressing home exercise programs and explaining treatment outcomes to patients and families. Comply with documentation standards, maintain an adequate caseload, and communicate regularly with supervisors, team members, and referring physicians. Attend required continuing education seminars and present educational programs to staff. Work per visit in the 1st shift in Middletown, CT, with a requirement for a Physical Therapy degree, Connecticut licensure, and 3–5 years of clinical experience.
