Physical Therapist - Per Visit

Develop a patient-specific plan of care incorporating safety, educational, and developmental needs while interpreting assessment data to set and revise goals. Administer therapeutic procedures and modalities, address home exercise programs, and explain treatment outcomes to patients and families. Maintain an adequate caseload adhering to departmental productivity standards and comply with professional documentation requirements. Communicate regularly with the Rehabilitation Supervisor, team members, and referring physicians. Attend continuing education seminars as required by licensure and present educational programs to Rehab staff. This role operates on a per-visit basis in the Days, 1st Shift at Middlesex Health in Middletown, CT.
