Physical Therapist - Per Diem

Conduct home health physical therapy assessments and evaluations to determine functional status, pain management needs, and mobility requirements. Develop and execute individualized treatment plans to restore function and improve patient well-being while coordinating with supervising nurses and physicians. Treat patients to relieve pain and maintain maximum functioning, then instruct families and caregivers on functional limitations and progress goals. Document each visit with progress notes, report treatment responses and condition changes, and submit evaluations per LHC policy. Perform duties in diverse home environments regardless of client demographics.
