Physical Therapist - Per Diem

Conduct home health physical therapy assessments for functional status, mobility, pain, and medical conditions to determine treatment plans. Perform interventions to improve patient well-being, relieve pain, and restore function while observing and reporting response to treatment and condition changes to supervising nurses or physicians. Instruct patients and caregivers on care plans and progress, complete progress notes per visit, and submit evaluations within state guidelines. Visit patients in diverse home environments, coordinate with the healthcare team, and report outcomes and projected care frequency. Requires current PT licensure, driver's license, and vehicle access; CPR certification preferred.
