Physical Therapist - Per Diem

Conduct individual patient therapy regimens while performing initial and ongoing systematic patient assessments. Set goals and develop treatment plans, monitoring patient response to modify sessions as needed. Teach and counsel patients and families, then participate in discharge planning including placement, education, and adaptive equipment. Supervise Physical Therapy Assistants and PT/PTA students, and promote continuous quality improvement by working cooperatively to solve patient-specific and facility-wide needs. Complete appropriate documentation per department policies.
