Job Summary
Per Diem Physical Therapist (PT) in Youngsville, PA. Provides direct resident therapy, conducts initial evaluations to establish care plans, interprets referrals, and develops/adjusts care plans within an interdisciplinary team. Maintains documentation for billing (MA, MC parts A & B) and collaborates with Medical Records, Nursing, and Financial Management to ensure compliant therapy records and billing. Requires PT licensure in PA and a PT degree from an accredited program; experience in long-term care and strong teamwork, communication, and regulatory knowledge are valued. Responsibilities include maintaining resident progress, discharge planning, supervision of PT Assistants as permitted, on-call availability within a 100-mile radius, and ongoing education for residents, staff, and families.
Required Qualifications
- Must be a graduate from a credited PT program
- Must be licensed to practice in the Commonwealth of Pennsylvania
