Job Summary
Provide in-home physical therapy evaluations and treatment plans to improve strength, balance, coordination, and mobility for patients in their homes. Develop individualized plans, deliver therapeutic exercises, educate patients and caregivers on exercises and safety, document care in the electronic health record, and collaborate with nurses, physicians, and other healthcare professionals to coordinate care while ensuring compliance with clinical and regulatory standards.
Required Qualifications
- Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy
- Current Physical Therapist license in the State of Connecticut
- Current BLS certification
