Job Summary
Physical Therapist per diem responsible for evaluating residents, developing and adjusting individualized treatment plans, and delivering therapy focused on mobility, gait training, strength, balance, and functional independence. Provides education to residents, families, and caregivers; collaborates with physicians, nurses, and rehabilitation staff; provides supervision to Physical Therapist Assistants; participates in care plan and interdisciplinary meetings; maintains accurate clinical documentation; trains staff in therapy techniques; maintains and troubleshoots equipment; delivers compassionate, resident-centered care aligned with FellowshipLIFE values. Requires graduation from an accredited Physical Therapy program, a New Jersey PT license, and a valid NJ Driver's License. Minimum one year of experience preferred; strong clinical assessment/treatment planning, communication, and ability to work independently within a team.
Required Qualifications
- Graduate of an accredited Physical Therapy program
- Current New Jersey Physical Therapist License
- Valid New Jersey Driver's License
- Minimum one year of experience preferred; new graduates are encouraged to apply
