Job Summary
Implement residents' personal care plans while supporting their physical, spiritual, emotional, and clinical needs. Focus on residents' strengths, preferences, and routines to provide comfort, companionship, and belonging within the continuum of care. Complete documentation to meet reimbursement and regulatory requirements. Deliver therapeutic services across multiple diagnostic groups including urinary incontinence, vestibular rehab, home safety assessment, movement disorders, and low vision. Work onsite with the interdisciplinary clinical team to address resident needs throughout their care journey.
Required Qualifications
- Must possess a valid and current state Physical Therapy license
- Degree and training must meet current Medicare and state requirements
- The ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
