Job Summary
Implement residents' personal care plans while focusing on their strengths, preferences, and routines to provide comfort and companionship. Collaborate with the interdisciplinary clinical team to address physical, spiritual, emotional, and clinical needs across specialty programs including urinary incontinence, vestibular rehab, and home safety assessment. Complete documentation to meet reimbursement and regulatory requirements. Work onsite with no additional travel, serving older adults across multiple diagnostic groups. Hourly pay range of $39.00 to $55.00 commensurate with experience.
Required Qualifications
- Valid and current state Physical Therapy license
- Degree and training meeting current Medicare and state requirements
- Ability to complete documentation to meet reimbursement and regulatory requirements
Desired Qualifications
- Therapeutic experience in older adults across multiple diagnostic groups
